The most common thing people write me is some version of this: "I wasn't even hungry and I ate anyway."
They usually say it with an apology attached. As if wanting food you don't physiologically need is a character flaw rather than a measurable event happening in your bloodstream.
Physical hunger comes on gradually, is felt as emptiness in the stomach, and is satisfied by any food. Hormone-driven craving arrives abruptly, is experienced as a mental preoccupation, targets one specific food, and often continues after you're full. The two run on different signals and respond to different fixes.

What is physical hunger, actually?
Physical hunger is a fuel signal. Your stomach empties, blood glucose drifts down, and the hormone ghrelin rises to tell your brain to go find calories.
It has a recognizable shape. It builds over 20 or 30 minutes rather than switching on. You feel it below the ribs, not behind the eyes. A boring dinner works just as well as an exciting one. And it stops when you've eaten enough, without you having to negotiate with yourself.
That last part is the tell most people miss. Physical hunger has an off switch that works on its own.
What is hormone-driven craving?
The other kind runs through the brain's reward circuitry rather than its fuel gauge.
Researchers separate "homeostatic" eating (calories you need) from "hedonic" eating (calories that feel good). The reward pathway responds to how appealing a food is, not to how empty you are. Ghrelin is involved in both, which is part of why the two get confused.1
Note the honesty caveat there. A lot of the reward-circuit mechanism, particularly the dopamine work, was established in rodents. The human evidence is mostly imaging and correlation, not cause.
Still, that imaging is interesting. A study of 109 European adults scanned people looking at food images after an overnight fast and again after a standardized meal. Higher fasting ghrelin was associated with stronger responses in visual attention regions when people viewed food. Higher leptin was associated with the prefrontal region involved in anticipated reward value, specifically for high-calorie images.2
Translated: people with certain hormone profiles literally look harder at food. That's not weakness. That's visual cortex.

How do you tell the two apart in the moment?
Five questions, ten seconds. I use these myself.

Three or more answers in the right column and you're dealing with the second kind. Which matters, because eating more food is a poor treatment for a signal that wasn't about food.
Does bad sleep really make you hungrier?
This is the single cleanest finding in the whole appetite literature, and it's over twenty years old.
Researchers at the University of Chicago ran 12 healthy young men through two days of sleep restriction and two days of sleep extension, with calories and activity held constant. After the short-sleep condition, leptin (the satiety hormone) fell about 18%, ghrelin (the hunger hormone) rose about 28%, and self-reported hunger rose 24%.3
The specific part is the good part. Appetite for calorie-dense, high-carbohydrate foods rose 33% to 45%. Not appetite for salad. Appetite for exactly the foods people describe craving at 10 at night.
Caveats, because they matter: 12 young men, controlled lab conditions, no energy expenditure measured. It's a small study that happened to be replicated many times since. If you want one behavioral lever for cravings, sleep is still the highest-yield thing on the list, and it's free.

Do menstrual cycle hormones change hunger?
Yes, and the research is messier than the internet suggests.
What holds up reasonably well: appetite-related measures do shift between the follicular phase (first half) and luteal phase (second half). A study of 12 women found acylated ghrelin was lower in the follicular phase than the luteal phase, and energy intake was higher on luteal-phase test days.4
A separate study of 20 healthy women measured resting energy expenditure and sweet taste preference on a representative day in each phase. Resting metabolic rate was modestly higher in the luteal phase, and sweet taste preference was higher too (16% versus 12%), alongside lower levels of FGF21, a liver hormone linked to sugar preference.5
Work in 9 women also found peptide YY and gastric emptying speed changed across cycle phases, with gastric emptying correlating with progesterone levels.6
Now the contradiction, which almost nobody quotes. A study of 10 physically active women compared appetite-regulating hormones after vigorous exercise in the follicular versus luteal phase and found no differences in appetite hormones, subjective appetite, or energy intake, even though ovarian hormones rose sharply in the luteal phase.7

Why does this get worse in perimenopause?
Because the hormone that was quietly helping starts leaving.
Estrogen influences energy balance on both sides of the equation. Reviews of the field describe estradiol acting on hypothalamic melanocortin signaling to promote satiety, and on energy expenditure. The decline across the menopausal transition is associated with a shift in fat distribution and a higher propensity to gain weight.8
Two honesty flags on that. First, a large share of the mechanism work (the melanocortin signaling, the brown fat thermogenesis) comes from mouse studies.9 Second, the observed weight change in women is confounded by aging, muscle loss, and sleep disruption all happening at once.
What women report is consistent enough to take seriously even where the mechanism is fuzzy: appetite feels different, evening cravings get louder, and the strategies that worked at 32 stop working at 47. Where does stress fit in?
Stress is the third lever, and it's the one people underrate because it doesn't feel like a hunger signal at all.
Elevated cortisol shifts food preference toward energy-dense options, and chronic stress keeps the reward pathway more responsive to those cues. What actually helps with the hormone-driven kind?
The interventions with the best evidence are unglamorous.
Sleep first. Given the ghrelin and leptin data, protecting seven-plus hours does more for cravings than any supplement on the market, including mine.
Protein early. Front-loading protein at breakfast blunts the afternoon and evening drift better than the same protein eaten at dinner.

Fiber and gut signaling. Viscous fiber slows gastric emptying and increases physical fullness.
